It is important to have as much information as possible before the initial appointment. This will enable us to best consider the needs of the adolescent and family.

The first step of the process is completion of the McCord Outpatient Referral Form.

This can be done in two ways.

  1. You may call the McCord Outpatient Assistant and she will record the information you provide. If you do not have some of the needed information, she may ask that you obtain this and call again or she can call you at your convenience.
  2. You may also complete the “Referral Form” which you can access by clicking here.
